For programs voluntarily seeking accreditation, the CAA requires documentation of compliance with its Standards for Accreditation, via completion and submission of an accreditation application and annual accreditation reports. The accreditation peer review process is comprehensive—looking at a program from many angles and against standards that have been developed by the profession as indicators of quality in the discipline of communication sciences and disorders. As a result, detailed documentation is required.
All forms are presented here in readable PDF files; MS Word versions are available for all accreditation reporting forms by contacting the Accreditation Office by e-mail at accreditation@asha.org. Please note: E-mail reminders and electronic versions of the forms for Annual Reports and Application for (Continued) Accreditation are sent to the program director approximately 3 months prior to the date of submission. In the fall of 2007, the CAA revised its reporting forms to align with the 2008 Standards for Accreditation and to integrate formats used in the Higher Education Data System (HES). To assist programs in achieving and sustaining quality through the accreditation reporting processes, the following revisions were made:
- addition of questions to allow programs to demonstrate compliance with 2008 CAA Accreditation Standards
- use of new question-and-answer formats to provide programs a range of responses in how they might demonstrate compliance
- addition of specific questions to document programs' compliance with standards for ALL modes of delivery (distance education and satellite)
- preparation of paper versions, which include all questions for both professional areas and for all modes of delivery, in anticipation of the on-line submission process through the HES in 2008-2009 (Note: Programs will be prompted only to complete questions specific to the type of program offered when using the HES; non-related questions will be blocked.)
