Speech-Language Pathology

Effective January 1, 2006, applicants for certification in speech-language pathology (CCC-SLP) must apply under the 2005 Standards for Certification in Speech-Language Pathology standards. Applications received after January 1, 2006, will be evaluated against the 2005 SLP certification standards.

• Speech-Language Pathology Clinical Fellowship (SLPCF) Report and Rating Form [PDF] (for individuals applying for certification under the 2005 Standards) Clinical Fellows and their mentors must use this form to report their Speech-Language Pathology Clinical Fellowship (SLPCF) experience.
• Clinical Fellowship Skills Inventory [PDF] – for documenting the skills upon which Clinical Fellows will be rated by their mentors.
• New Applicant Dues and Fees Schedule [PDF] – complete and submit with your payment and application.
• Journal Selection Form [PDF] – complete and submit with your application.

Note: The implementation language in the new speech-language pathology standards makes reference to the Knowledge and Skills Acquisition Summary (KASA) form for Certification in Speech-Language Pathology [PDF] that will be used by students and programs to track the student's acquisition of the requisite knowledge and skills for ASHA certification.

Audiology

Applications received on or after January 1, 2008, will be evaluated against the 2007 audiology certification standards.
