Applicants for reinstatement who submit the application on or after January 1, 2008*
Applicants must meet the 2007 Audiology Certification Reinstatement Policy. The 2007 reinstatement policy is as follows:
If lapsed 5 years or less – Submit a reinstatement application form, reinstatement fee, and evidence of professional development** hours based on the number of years lapsed, i.e., 1 year = 10 hours of professional development; 2 years = 20 hours; 3 to 5 years = 30 hours. Professional development activities must have been completed no more than 3 years prior to submission of the reinstatement application form.

If lapsed more than 5 years – Submit the following documentation:
- Reinstatement application form
- Appropriate fees
- Confirmation of completion of a 12-week FTE clinical experience supervised by an ASHA-certified audiologist, completed no more than one year prior to the submission of the application for reinstatement
- Confirmation of completion of 30 hours of professional development. Professional development must have been completed no more than 3 years prior to the submission of the reinstatement application.
- Confirmation via an official notification from the Educational Testing Service (ETS) of successful completion of the Praxis Examination in Audiology with a score of 600 or above, achieved no more than 5 years prior to submission of your reinstatement application. This requirement must have been completed before the reinstatement application is submitted.
* Important: Individuals applying for reinstatement as of January 1, 2012 must submit an official transcript confirming the receipt of a doctoral degree.
Please note that all requirements for reinstatement must have been completed prior to submission of the reinstatement application. Incomplete applications will be returned for re-submission once all requirements have been met.
** Types of professional development activities that are acceptable for reinstatement of certification
Professional development activities that are acceptable for reinstatement must address the provision of clinical services specific to the professions of speech-language pathology and audiology.
Applicants for reinstatement should choose courses and/or activities that will enhance knowledge and skills that are included in the Scopes of Practice and the knowledge and skills required in the Certification Standards of the profession in which reinstatement is being sought.
Acceptable activities could include, but are not limited to, review courses that applicants can use to "brush up" on current knowledge and skills, or to learn new knowledge and skills, or that may not have been covered in the applicants' academic training and are now part of the scopes of practice.

Types of professional development activities that are not acceptable for reinstatement of certification
Courses and activities that are in related areas, are general in nature, or are not related specifically to the practice of speech-language pathology and/or audiology will not be accepted for the reinstatement professional development requirement. Examples of courses that would not be accepted include, but are not limited to, CPR, relaxation techniques, languages, or computers.
