These forms are for use by authors whose manuscripts have been accepted for publication in one of ASHA's journals.
Within 5 business days of receiving your e-mailed decision letter, please submit both of the administrative forms.
The author information form is a Microsoft Word electronic form, but the copyright form is a PDF file and will require Adobe Acrobat® Reader (Version 5.0 or greater) to be viewed and printed. This reader is free and may be downloaded from the Adobe Web site.
Author Information Form
The corresponding author must provide his/her name and contact information. A backup contact must also be provided.
The corresponding author should complete the author information form and e-mail it to tholmes@asha.org.
Copyright Form [PDF]
This form is required by ASHA journals and transfers copyright to the American Speech-Language-Hearing Association.
Each author should complete both pages of the copyright form and fax to the attention of Taniza Holmes-Craggette at 301-296-8587.
The fax cover sheet should include the accepted manuscript number and/or the first author's name.
When all forms and permissions are received, (including possible multiple copies of the copyright form), your manuscript will be forwarded to the production editor.
