Change Name, Address or Update Degree Information

 E-mail changes to the National Office at nsslha@asha.org and include the following: 

• In the subject line of the message, type "Address Change," "Name Change," or "Degree Change" as a reference.



• Provide your name and NSSLHA membership number (if available)

If you are submitting a:

•  Name Change: Please type your full name as you would like it to appear on all NSSLHA and ASHA mailings.



•  Address Change: Please indicate whether the new location is a Home, School, or Work location.



•  Degree Change: Please provide a degree designator (AuD, BS, BA, MS, MA), the area of study (Aud, CSD, SLP), the expected date of graduation, and the institution name (or university code).

If you e-mail the National Office, you will receive a confirmation reply e-mail after your information is updated.
